VITAS INNOVATIVE HOSPICE CARE – MIAMI DADE 2005-Present
Hospice Chaplain
Responsibilities include:
- Conducting assessments on patients and their families
- Determining their spiritual and emotional need(s)
- Offering support that meets their needs
- Updating the hospice team on patient status
- Documenting all care according to policy
- Preparing and conducting memorial services/funeral services
- Providing emotional support to the hospice team
- Attend death visits and follow-up required protocol
- Bereavement follow-up calls and bereavement visits as needed
- Mentoring CPE Students
Services are provided to predominantly adult and geriatric patients in nursing homes, but also responsible to visit patients in the hospital settings, and at the home setting. The focus is to work within a multi-disciplinary hospice team to provide pastoral/spiritual care to hospice patients in nursing homes. Also participates on call services, provides spiritual support to the hospice staff, and submit paperwork in a timely manner.
