Financial Aid Verification Documents

If your financial aid application is selected for verification by the Department of Education, you will receive an email from our financial aid servicer stating which verification documents you need. Typically, we will contact you as well.

The automated email will indicate what verification documents you need to submit to us. You can submit them via fax at 816-256-8471, or by uploading them directly to the VFAO at https://cityvision.vfao.com/formsdocuments/trackingdocumentsschool/default.aspx.

You can download the verification documents from the Downloadable Forms page under Forms & Documents on our online financial aid office site: https://cityvision.vfao.com/formsdocuments/downloadableforms/default.aspx

Note that if you requested to submit a "Statement of Educational Purpose" form, you must present your identification, such as a driver's license, in front of a notary, in order to complete. You will need to mail the original (not a copy) to us at:

Make sure to keep a copy for your own records.

We need the original so we can confirm that it was notarized.
